Home Health Physical Therapist OPENINGS : Per Diem

Mare Island Home Health Inc. currently provides services to the following cities in the following counties:

Solano County
Napa County
Yolo County
Contra Costa County

We are currently aiming to expand our network to reach patients who require home health needs in Yolo County, focusing on the Davis, Woodland, and West Sacramento areas.

Initial Requirements:

Current license to practice in CA
Current CPR certification
Current CA driver's license plus proof of car insurance.
Minimum one year clinical experience (two+ years preferred)
Bi/Multi-lingual skills are highly desired (e.g. Spanish, Russian, Tagalog, etc.)

Required experience:

Physical Therapist: 1 year

Responsibilities

Provide physical therapy services to patients in their homes, focusing on patient care and rehabilitation.
Utilize knowledge of physiology, anatomy, and physical therapy techniques to create personalized treatment plans.
Incorporate medical terminology into documentation and communication with healthcare professionals.
Implement discharge planning strategies for smooth transitions post-treatment.
Collaborate with interdisciplinary teams to ensure comprehensive care, especially in acute care settings.
Adapt care plans to meet the unique needs of pediatric patients, emphasizing a holistic approach to therapy.

Pay: $140,000.00 - $164,000.00 per year
