Title: Information and Assistance Specialist - Tagalog Speaking

Reports To: Social Services Program Manager
Summary: Assists in the navigation of the home and community-based long-term care system; provides assessment, information, referral and assistance to older adults, adults with disabilities, and caregivers; performs follow-up to ensure delivery of services; collects, updates and shares information on community resources.
Essential Functions
- Conducts basic needs assessments of older adults and disabled adults to provide them with information and/or referrals to agencies which offer services that match their needs.
- Assists consumers in accessing needed services and advocates on their behalf when the requests do not require short-term casework level of assistance (e.g. contacting agencies on behalf of clients, filling out forms, writing letters, or making phone calls to set up or confirm appointments).
- Provides follow-up with consumers and/or agencies to ensure that needed services were received by consumers.
- Documents services according to program protocol and submits monthly service logs and reports.
- Develops and implements outreach strategies in collaboration with the Citywide I&A Supervisor.
- Conduct regular outreach and presentations in Filipino community settings (e.g., churches, community centers, cultural events) to raise awareness of available resources.
- Establishes contacts and maintains good working relationships with key personnel in other agencies, including community-based service providers in the district and citywide providers.
- Participates in meetings and professional development training sessions as assigned.
- Performs other duties as assigned.
Qualifications
- Bachelor’s degree related to human services, or high school graduate with two years of related work experience.
- Experience providing health, social, or aging services, particularly in diverse communities.
- Familiarity with community-based resources and social service systems.
- Culturally sensitive and empathetic toward the needs of older adults and adults with disabilities, especially within ethnic and minority populations.
- Good problem-solving skills and ability to follow through.
- Excellent verbal and written communication and interpersonal skills.
- Bilingual and bi-literate in English and Tagalog required.
- Proficient in MS Office and the Internet.
Must be able to travel via public transportation or personal vehicle for the performance of job duties. If driving, must have and maintain a valid CA driver license, satisfactory driving record, and auto insurance coverage as specified in Self-Help's policy.
