Position Summary:
We are seeking a Clinical Case Manager / Wraparound Facilitator to coordinate health, behavioral, and community-based services for individuals receiving Supported Living Services (SLS). This role serves as a key liaison between clients, families, Direct Support Professionals (DSPs), clinical providers, and Regional Center staff. The ideal candidate will be detail-oriented, skilled in advocacy, and passionate about improving the quality of life for people with developmental disabilities.
Key Responsibilities:
Medical & Clinical Coordination
- Schedule and attend client medical, psychiatric, and dental appointments
- Advocate for clients with providers and ensure follow-up care is completed
- Maintain accurate medical records and medication documentation
- Collaborate with pharmacies, physicians, and durable medical equipment providers
Behavioral Health Support
- Track behavioral service appointments and implement Behavior Intervention Plans (BIPs)
- Review behavior data, monitor psychiatric progress, and support staff with implementation strategies
Wraparound Facilitation
- Serve as Wraparound Facilitator coordinating services across:
- DSPs and ILS Instructors
- Behavioral clinicians and medical providers
- Regional Center Service Coordinators and other stakeholders
- Lead wraparound team meetings to ensure alignment with IPP goals
- Ensure individualized, culturally responsive support across all services
Documentation & Compliance
- Maintain detailed client records in Electronic Health Records (EHR) system (SmartCare)
- Track documentation deadlines for IPPs, quarterly reviews, assessments, and incident reports
- Ensure compliance with Title 17, DDS, HIPAA, and internal quality assurance standards
- Organize digital and physical client files; prepare reports and audits
Qualifications:
Required:
- Bachelor’s degree in Nursing, Social Work, Public Health, Psychology, or related field
- 2+ years’ experience in developmental disabilities, case management, or health coordination
- Strong understanding of HIPAA, EHR systems, and clinical documentation
- Valid CA driver’s license and reliable transportation
- Excellent communication, time management, and coordination skills
Preferred:
- LVN, Medical Assistant, or Bachelors Degree in a related field
- Familiarity with Title 17, DDS systems, and Regional Center services
- Bilingual (Spanish, Tagalog, or other languages) is a plus
