Job Title: Case Manager/Social Worker
Location: Honolulu, Hawaii 96816
Job Type: Full-Time
Salary: $28 - $31/hr.
Schedule: Monday – Friday | Day shift
Key Responsibilities:
- Conduct comprehensive psychosocial assessments for individuals and families.
- Develop and implement individualized care and discharge plans.
- Provide counseling and crisis intervention to clients dealing with emotional, mental, or physical challenges.
- Collaborate with interdisciplinary healthcare teams, community agencies, and support services.
- Advocate for patients’ rights and access to resources, including housing, healthcare, and financial assistance.
- Maintain accurate and timely documentation in client records.
- Facilitate support groups, education sessions, and outreach programs as needed.
- Assist clients in navigating social services, benefits, and community-based programs.
- Respond to emergencies and provide on-call support if required.
Qualifications:
- Bachelor’s or Master’s Degree in Social Work (BSW or MSW) from an accredited program.
- Current and valid Hawaii State Social Worker license (LSW, LCSW preferred).
- Minimum 1–2 years of experience in a healthcare, community, or clinical setting.
- Strong understanding of local and federal social service resources.
- Excellent communication, advocacy, and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in documentation, case management software, and Microsoft Office Suite.
Preferred:
- Bilingual or multilingual skills (Hawaiian, Tagalog, Japanese, or Pacific Islander languages).
- Experience working in diverse cultural settings.
