Reporting to the Clinical Supervisor of the team, the Medicare Care Management Field Coordinator supports the health and well-being of San Francisco Health Plan (SFHP) members by addressing health needs and social drivers of health, both individually and across communities. As part of the Care Management team, they work directly in the community with those who benefit most from SFHP services. Often from the communities they serve, Field Coordinators are trusted partners who help bridge cultural and language gaps, improve access to care, and connect members to needed services.
You will help improve health equity, support culturally responsive care, and empower individuals and communities through advocacy, education, and support. Field Coordinators also help reduce emergency visits and hospitalizations, and increase access to integrated medical, behavioral, and dental care. Your role will involve collaboration with care teams, providers, community partners, and agencies to enhance member care and outcomes.
This is a hybrid position, based in our Downtown San Francisco office. You will be working in the field within San Francisco up to five days per week, with one of these days typically in the SF office or remotely.
Salary: $30 - $39 per hour
What You Will Do
- Support members in completing Health Risk Assessments (HRA) by phone or in-person, in their preferred setting.
- Build respectful, trusting relationships; explain Care Management services and benefits.
- Assist members in accessing care, including scheduling, post-discharge follow-up, and attending appointments.
- Help with applications for eligible programs; coordinate communication among members, caregivers, providers, and community-based organizations (CBOs).
- Foster inclusive partnerships with providers and agencies to reduce cultural and socioeconomic barriers.
- Stay informed about local resources; conduct screenings, identify social needs, and follow up on referrals.
- Participate in interdisciplinary care team meetings and contribute to personalized care plans.
- Support members in understanding and following their care plans.
- Share observations from community visits to help the care team respond to members' evolving needs.
- Encourage members to actively participate in their health, addressing barriers to care and services.
- Document activities and collaborate with care transition teams to support continuity of care.
- Deliver culturally relevant health education on chronic disease prevention, nutrition, and physical activity.
- Work with other staff and departments to support comprehensive, person-centered care.
What You Will Bring
- Associates degree or higher, Community Health Worker program certification preferred.
- One year of Field Coordinator, Care Navigation, Community, or Public Health work experience
- Bilingual language skills preferred: Chinese, Spanish, Russian, Vietnamese, or Tagalog
- Knowledge of San Francisco County community resources
- Knowledge of Dual Special Needs Plan (D-SNP) benefits preferred
- Experience with medical, behavioral, and social risk factors, including chronic conditions and common treatments
- Cultural awareness and sensitivity to the needs of diverse communities
- Collaborative mindset with the ability to engage members, families, care teams, and community partners
- Excellent customer service skills, in-person and by phone
- Strong listening, advocacy, and communication skills to support member care goals
- Non-judgmental, compassionate approach to working with others
- Work respectfully with people from varied backgrounds and lived experiences
- Clear written and verbal communication in English
