As a member of the Community Jobs Program (CJP) the Case Manager works with the Program Manager, Trainer/Job Coaches and Job Developers in an effort to increase CalWORKs participant’s employability through a job readiness and job preparation program that includes vocational skills training and paid work experience.
Job Duties
Essential Job Duties include, but are not limited to:
- Provide culturally competent behavioral health case management services, including outreach, referral, at program sites and/or in the field.
- Conduct Intake and Enrollment of participants referred by HSA
- Develop Individual Employment Plan with each participant
- Identify Job Readiness Level
- Provide one-to-one assistance for any employment/vocational barriers
- Assist participants with problem resolution, provide resource information to help them obtain supportive services to address barriers to employment, such as domestic violence or childcare as needed
- Provide referrals for participants who need additional services, such as behavioral health or domestic violence counseling, SSI referral, or social work interventions by working collaboratively with HSA to accommodate the needs of the participant
- Provide referrals for participants who could benefit from further education or training
- Maintain weekly communications with participants
- Communicate with HSA staff on an ongoing basis, reporting participants absences within two days of occurrence
- Identify community resources and establish referral system, as well as serve as a resource to RAMS and HSA staff
- Develop and maintain clear, open, timely, cooperative, and diplomatic communications and working relations with all staff, consumers, and partner agencies
- Complete and maintain timely administrative and case management records and documentation related to client care, in accordance to standards specified by RAMS policies, clinical standards, and oversight agencies (e.g. HSA, CalWORKs, etc.)
- Participate in and facilitate various educational & training workshops relating to behavioral health issues, technical support/training, and psycho-educational topics for clients, HSA program staff, other professionals, and/or the community-at-large, as deemed necessary
- Participate in case conferences, program team and staff meetings, in-service trainings and other staff development activities
- Participate in case conference meetings with HSA staff to make next step recommendations, referrals for ancillary, other supportive services, or address concerns and document recommendations
- Perform other duties as assigned
Minimum Required Qualifications
- Bachelor’s degree in Psychology, Social Work, or other health & human services related fields from an accredited institution OR two years of experience providing similar types of services, required. Substance Abuse Counselor Certification, highly preferred
- At least one year of experience in working with the indigent & underserved populations (including those with mental health, substance abuse, and/or co-occurring substance abuse disorders, low income families), highly preferred
- Knowledge of San Francisco community resources/services (including health, mental health, substance abuse, housing, family support and resources, etc.), highly preferred
Other Requirements
- Familiarity with various counseling strategies and wellness recovery principles in working with clients with mental health, substance abuse, or co-occurring substance abuse issues
- Strong commitment to the philosophy, goals, & mission of RAMS in providing community-based, culturally competent, and consumer-guided mental health and vocational services to the community
- Commitment to engaging in professional development activities such as trainings, continuing education seminars, case conferences, and reviewing up-to-date material, etc., as related to vocational training strategies for the population of CalWORKs
- Strong interpersonal skills and ability to work effectively, function independently, and interact professionally with a diverse, multi-cultural, and interdisciplinary staff of all levels
