Job Summary

Fathers To Founders is seeking passionate, community-rooted Community Health Workers (CHWs) to provide person-centered support for individuals facing complex life challenges—including homelessness, reentry, behavioral health, substance use, and chronic conditions. As a CHW, you will act as a trusted advocate and bridge between Medi-Cal members and vital health and social support systems, with a special focus on culturally resonant care.

This position is part of our Enhanced Care Management (ECM) team, working directly with Lead Care Managers and Clinical Consultants to deliver whole-person care. CHWs help clients overcome barriers, access services, and stay engaged in their care plans through trauma-informed outreach, resource navigation, and peer support.

Key Responsibilities

Conduct outreach in community settings (shelters, encampments, SROs, public libraries, jails, etc.) to connect with high-need individuals.
Engage clients using motivational interviewing, trauma-informed care, and harm reduction strategies.
Support completion of ECM assessments, care plan follow-ups, and goal setting.
Assist clients in navigating Medi-Cal, housing, food, behavioral health, and substance use recovery systems.
Provide field-based peer advocacy and accompaniment to appointments as needed.
Translate complex medical and social information into culturally and linguistically accessible language.
Collaborate with healthcare providers, social service agencies, and internal team members to ensure coordinated care.
Participate in case conferencing and team-based care coordination activities.
Maintain timely, accurate documentation using electronic tools or forms required by ECM standards.

Qualifications & Requirements

Lived experience with homelessness, justice involvement, recovery, or similar challenges is strongly encouraged and valued.
Prior experience in outreach, peer support, case management, or community advocacy is preferred.
Strong interpersonal skills and ability to build trust with individuals from diverse backgrounds.
Comfort with public speaking, storytelling, or group facilitation is a plus.
Understanding of Medi-Cal, social services, or behavioral health systems is beneficial.
Basic documentation and data collection skills required; training provided.
Must be dependable, communicative, and able to work both independently and within a team.
Reliable phone, internet, and transportation access preferred.
Bilingual skills (e.g., Spanish, Tagalog, Cantonese) are highly desirable.
